Remove teammate access from conversations

Problem
Admins and managers cannot remove individual teammates from conversation visibility, even after they've been initially looped in to respond. Teammates retain view access to the conversation indefinitely, creating potential compliance risks.

Enable admins and managers to revoke individual teammate access to specific conversations, independent of their original loop-in status.
