Increase visibility of channel names in sequence creation modal

Problem
The Sequence Create/Edit modal is narrow and non-resizable, causing From channel names to be truncated with ellipsis, making it difficult to select the correct channel when many channels exist with similar names.

Expand or make resizable the sequence creation modal so channel names are fully visible when selecting the From channel.
